All fees, including co-payments and co-insurance payments, are due at the time of service. Payment is accepted in the form of cash or credit card. Checks are accepted up to $100.00.
Private Insurance: Dr. Carter has a concierge practice. In order to provide the highest possible level of care, she will only accept certain insurance plans on a case-by-case basis. This allows Dr. Carter to offer 1 hour consultations, same or next-day appointments, minimal in-office wait time and 24 hour access via phone, email or pager. Please call the office with your insurance information if you would like to know if Dr. Carter can accept your plan. If you are in need of an injection procedure, Dr. Carter may be able to negotiate a special rate with your insurance company in order to accept assignment.
|Medicare:||Sorry, the practice does not accept medicare at this time.|
|No-Fault:||No Fault patients must pay upfront and the office will provide an itemize receipt to submit to your No Fault insurance for reimbursement. The office does not accept assignment on No Fault.|

24 Hour Cancellation Policy
If you miss your appointment or cancel your appointment without advanced notice, it may prevent Dr. Carter from helping other patients in need. In consideration to other patients and to our practice, kindly give 24 hours notice if you are unable to keep your appointment. Noncompliance with this policy will result in a late-cancellation fee before scheduling another appointment.
If you are a new patient OR if you have not been seen by Dr. Carter in over a year, please download and complete the following forms prior to your visit. You may also call the office to have these forms faxed or mailed to you. In order for Dr. Carter to maximize her time spent with you on the day of your visit, it is recommend that you complete these forms in advance. If you are unable to do so, then please arrive 20 minutes prior to your appointment to complete these forms.
Your initial consultation will begin with a complete spine and orthopedic examination.
Dr. Carter will also review any records that you have pertaining to your condition. Your x-ray, MRI and other diagnostic test results will be reviewed with you. Dr. Carter will discuss the nature of your condition and will answer any questions that you have. You will be given a comprehensive treatment plan based on your condition, consisting of dietary and lifestyle changes, activity modification, medications and/or injection procedures.
In most cases, injection procedures can be performed on the same day as your consultation. Please see our pre-spinal injection procedure instructions to find out what you should do prior to having a spinal injection procedure.
Please be on time for your appointment. We do our best to see all patients within 15 minutes of their scheduled appointment time. If you arrive late, we cannot guarantee your full allotted treatment time nor can we guarantee that you can still be seen that same day. Please go to or policies and forms section to download the appropriate forms to fill out in advance of your appointment.
Please remember to bring:
- Any prescriptions given to you by your referring physician.
- Relevant imaging studies or diagnostic tests (X-rays, MRIs or CT scans). The actual films/CDs are preferable, however if they are unavailable then a report should suffice.
- Prior medical records pertaining to your condition (including operative or procedure reports)
- Your insurance card.
- Your completed patient registration and patient questionnaire forms.
